Incident Narrative
Two employees were transferring liquid from a nearly-empty tote to a nearly-full tote using a forklift. As they were performing this task, the tote on the forklift rotated slightly and then slipped off the forks, striking the injured employee assisting on the ground. The employee began having breathing issues and was hospitalized with eight rib fractures.
