Incident Narrative
When a forklift operator went to lift a pallet with the forks, the pallet slipped and struck a stack of pallets on the top rack in the aisle. The stack of pallets fell from the top rack to the floor below and struck an employee who was walking near the rack. The employee then fell across the room and struck the wall with the right side of his body. The employee sustained dislocations of his right shoulder and knee.
