Incident Narrative

An employee removed a floor plate and reached into the opening to manually flip a limit switch on the arbor turntable. The employee then asked a machine operator to turn the table rotate switch. The table rotated and the employee's arm was caught between the table floorplate and the locking pin cylinder block. The employee sustained multiple fractures to their arm and was hospitalized. The machine was not locked out/tagged out at the time.
