Incident Narrative

An employee was working from a 6-foot step ladder to look for a possible concrete wall opening to run a wire. The employee lifted up a ceiling tile and turned his torso to look at the area above the ceiling tiles. The ladder shifted and the employee fell approximately 4 feet to the concrete floor, striking their hip and lower back. The employee sustained a spinal fracture in their lower back.
