An employee was using an electric pallet jack to make a delivery at a customer location when the jack malfunctioned and continued reversing after the employee stopped pressing the reverse button on the handle. The jack and loaded pallet struck his leg and he fell from the liftgate onto the street. The employee struck his right elbow, shoulder, upper back, and chest on the street and was hospitalized with fractured ribs.
