Incident Narrative
An employee was painting the break room wall while standing on the third step of a 6-foot A-frame ladder. The employee was stepping down from the ladder so he could relocate it to complete more painting. The employee missed a step and fell from the ladder to the concrete floor. The employee struck his head/neck on a nearby swivel chair and struck his left hip on the concrete. The employee sustained multiple compression fractures to his spine, including the C7 vertebra, T3-5 vertebrae, and the third and fourth sacral segments. The employee was hospitalized.
