Incident Narrative
An employee was standing in a concrete vestibule at a facility while reviewing renovations with a contractor. A catwalk that was previously adjacent to the vestibule had been removed. The concrete vestibule floor collapsed while the employee and contractor were standing on it, and they fell approximately 5 feet to the floor below. A concrete wall of the vestibule then fell on top of them. The employee suffered back and neck injuries and was hospitalized.
