Incident Narrative
An employee was standing on an 8-foot ladder about halfway up the rungs (approximately 5 feet off the floor) to reach the ductwork to make the cut with a reciprocating saw. The unit snapped off and swung down, striking the large HVAC unit mounted to the concrete slab and bouncing back into the employee s ladder. The employee jumped off the ladder and landed on both feet on the concrete floor, causing a fracture to the employee s lower left leg.
