Incident Narrative

At 11:09 p.m. on January 8, 2025, an employee was stabilizing a combo tipper in a fresh meat cooler by placing a shim under its base plate. A forklift was being used to hold the plate a few inches off the floor. The plate shifted and the employee's right hand was caught between it and the floor. The employee suffered partial amputations to the index and middle fingertips.
