Incident Narrative
An employee was spotting while a forklift unloaded cooling modules from a truck. During the process, the forklift's forks became lodged between a cooling module's frame and the truck bed. The spotter went to remove plastic blocking their view of the forks and the module slipped off the forks, pinning the employee's arm against the truck bed.
