Incident Narrative
On December 16, 2024, at approximately 8:21 AM, an employee was operating a forklift, picking up boxes in the bulk-floor items area. The employee dismounted the forklift and went to move or dislodge boxes of 85-inch televisions from a bin. The employee was pulling on the banding of the boxes when the banding snapped, causing the employee to fall backward onto the floor. The employee landed on their left hip and sustained a fractured hip requiring hospitalization and surgery.
