Incident Narrative
On January 29, 2024, an employee was loading a pallet onto a truck using an electric pallet jack. The pallet became stuck on the pallet jack's forks. The employee dismounted the pallet jack and tried to get the jack loose by turning the steering column back and forth while turning the throttle forward. The pallet dislodged and struck him. He fell to the floor and suffered fractures to his ribs and right wrist. He was hospitalized.
