Incident Narrative
An employee was installing sprinkler heads from a 10-foot A-frame ladder, working from the step below the top of the ladder. The employee was finishing the installation of the last sprinkler head when the ladder shifted as they reached overhead. They fell approximately 9 feet to the concrete floor, resulting in fractures to their left arm, wrist, tailbone, and back.
