An employee was installing a 62-pound insert into a mold on the toolroom table. The insert got stuck when the employee pushed it into position. The employee tapped on it with a mallet and the insert fell onto his left index and middle fingers. Both fingers were fractured and severely lacerated. The employee was hospitalized and required surgery.
