Incident Narrative

An employee was using a forklift to load a unit of material onto a trailer. When the employee set the material on the rear of the trailer and pushed forward to complete the load, the weight caused the back of the pickup truck to lift. This caused the forklift to push the pallet and it pressed against the employee's hand, resulting in the partial amputation of his right index finger.
