Incident Narrative
An employee was raising a dock door while standing on a powered industrial truck (PIT). While pushing up on the door with his left hand, he pulled on the control handle of the PIT which caused the PIT to move toward the dock door. The employee's left middle fingertip was caught and amputated between the head rack of the PIT and the bottom panel of the dock door.
