Incident Narrative
An employee was ascending the ladder on a rail car to the top deck when he climbed past the deck. While transitioning into the rail car his foot was placed on the bridge plate instead of the rail car and the plate moved, causing him to lose his balance. The employee fell 12 feet, striking the rail car cut lever and the ground. The employee sustained lower back and head injuries.
