Incident Narrative

An employee was working on reinforcing steel rebar while wearing a full body harness with a retractable hook. The bar that the employee was hooked to bent and the fall protection slid off, causing the employee to fall 12-15 feet to the ground. The employee suffered a fractured skull, damaged ear canal, broken bones in the face, a broken collar bone, three brain bleeds, a collapsed lung, and broken ribs.
