Incident Narrative

An employee was helping to change a hose on an excavator. He was in the raised bucket of a powered industrial vehicle (PIV) when his 15-inch adjustable wrench slipped off a nut. He lost his balance and fell about 8 feet to the ground. The employee sustained injuries to their neck and back, a laceration to his head, and fractured ribs. A fall arrest system was not used at the time of the fall.
