Incident Narrative

An employee was in a warehouse loft. There was a railing along the outside of the loft with a gated section that could be opened so items could be loaded into the loft. After loading items, the employee fell through the gate and landed on the concrete floor about 13 feet below. The employee suffered injuries to the head, back and ribs.
