Incident Narrative

A forklift was being used to lift the ends of a bundle of concrete reinforcement rebar by the wire holding the bundle together. An employee was positioning wood dunnage under the bundle when the wire failed. The bundle fell on the employee's right hand, breaking the index and middle fingers and causing a 2-inch laceration at the base of the broken fingers. The employee was hospitalized.
