Incident Narrative
An employee was installing a blower part to a flat glass machine. There was a jack under the blower and a 2 x 6 was under the jack. The employee was holding the 2 x 6 and operating the jack when the top of the jack caught on a flange and the blower slipped off the top of the jack. The tip of the employee's left thumb was crushed between the blower and the wood and it was amputated.
