Incident Narrative

The injured employee was training on an electric pallet jack along with other employees. She got off the pallet jack to troubleshoot and asked another employee to help her move it. When the pallet jack moved forward it pinned the injured employee against some product. The employee was hospitalized with a fracture to their right foot and required surgery.
