Incident Narrative
An employee was making a repair to a fan that was secured to a shipping pallet with bolts. He removed the shipping bolts and stood the fan vertically on the pallet when he lost his grip and his right index fingertip was caught between the fan flange and the shipping pallet. He sustained a fingertip amputation.
