Two employees were connecting braces to joists. They were in a scissor lift with the platform approximately 40 feet high. They started moving the scissor lift forward. The left front tire fell into a floor hole causing the scissor lift to turn over. The employees fell to the concrete floor and both employees sustained fractures.
