Incident Narrative

An employee moved from a trench box to a man hole box to clean around a sump pipe with a shovel. An excavator was moving a steel plate used for shoring in the man hole box. The steel plate fell onto the bridge of the employee's foot, dislocating their ankle and fracturing several bones on top of the foot.
