Incident Narrative
Upon completing the threading of a piece of 1 pipe an employee began to wrench on a 90-egree fitting using a pipe threading machine as a vise. The employee inadvertently stepped on the automatic pedal of the pipe threader, engaging the equipment while they were installing the fitting on the opposite end of the pipe. The employee s finger got caught between the wrench and the stabilizing bar, causing a fingertip amputation.
