Incident Narrative

An employee was about 8 feet high on a 12-foot ladder, securing a pipe with a duct lift and chain fall in preparation for pipe installation. The employee was adding a second chain fall and installing an anchor when a cut section of the pipe shifted and struck the ladder. The employee fell from the ladder to the ground, suffering injuries to the head, back, and left side.
