Incident Narrative

An employee was on a 10-foot A-frame portable step ladder positioned in front of a forklift. The forklift mast moved forward and knocked the employee off the ladder. He fell to the floor about 7 to 8 feet below, suffering a broken ankle (which required surgery) and an elbow injury.
