Incident Narrative
An employee was felling a tree with a chainsaw when the chain contacted a yellow wedge that was jammed in the center of the tree. The chainsaw walked along the wedge and kicked back out of the tree. The chainsaw then pivoted along the rear handle and pulled the employee's left hand allowing the chain to contact and lacerate his left forearm. The employee was hospitalized and required surgery.
