Incident Narrative

An employee was using a 4'x8' 3/4" MDO plywood to construct an 8' trash bin. The employee was using the plywood as a saw horse to put another side on the bin. He went to cut the wood using an electric circular saw and the vibration caused the wood to fall which caused the saw to kick back and amputate his left index finger below the second knuckle and cut his left thumb.
