Incident Narrative
Employees were aligning a 10-inch bolt on a newly purchased 10-inch tuber machine. The injured employee's hand was caught between the flat bolt head and the frame of the machine as the bolt was torqued. The employee's middle finger was crushed when the rod was turned in order to connect the barrel section to the extruder. The employee's fingertip was surgically amputated.
