Incident Narrative

On November 17, 2020, employees were replacing a steel core. The injured employee was releasing air from the airshaft by depressing the air valve while a forklift operator was removing the airshaft from the steel core. The injured employee's right index finger was pinched between the steel core and the airshaft causing a partial fingertip amputation.
