Employees were unbolting a 12-14-foot-tall concrete-filled bollard that was leaning against an overhead door. As they went to move the bollard off the door and onto a wall, the bollard rolled and slid, landing on the injured employee's foot. They sustained a crush injury to the fourth and fifth toe.
