Employees were at the edge of an excavation at a construction site waiting for the excavator to remove a stuck pipe. As the pipe released from the excavation, it ricocheted off the trench box and struck the injured employee's face. The employee sustained facial injuries to the nose, jaw, and chin that required surgery.
