Incident Narrative
An employee was preparing a bushel corn bin for filling and was ensuring that residual product was removed from all gates. One gate had remaining product in it and as the employee lifted the modification tab, the slide gate began closing. The modification tab contacted the outer edge of the slide gate enclosure, amputating their right index fingertip.
