Incident Narrative

A maintenance employee was working on a bailer compactor. He inserted a screwdriver into the chain to give it slack to reset the chain to the sprocket. The screwdriver slipped, and the chain came down catching his left ring finger between the chain and sprocket and amputating a small part of the fingertip.
