Incident Narrative
An employee was removing a chock used to stabilize a 25,000-pound pressure vessel. After a forklift rolled the vessel away from the chock, the tine of the forklift struck the vessel, causing it to roll forward just as the employee reached down to remove the chock. The employee's left hand was caught between the chock and the vessel, resulting in amputations to the index finger above the second knuckle and the middle finger below the second knuckle. The employee also sustained a ring finger fracture.
