Incident Narrative
An employee was installing a joist seat at roof level while standing on an extension ladder that was placed on metal decking. The ladder began slipping and the employee fell approximately 7 feet to the ground surface. As he fell, his foot was caught between the rungs of the ladder. The employee sustained a broken right ankle requiring hospitalization and surgery.
