Incident Narrative
An employee was operating a forklift to load empty racking systems onto the bed of a trailer. The employee parked the forklift on the dock plate and entered the trailer to adjust one of the racks. The parking brakes failed and the forklift began to roll, pinning the employee between the forklift and a rack in the trailer. The employee sustained fractured ribs.
