Incident Narrative

An employee was operating a tow truck when he noticed one of the chains needed to be tightened. The employee pulled off to the side of the highway on the sloped ramp and put a 4x4 block under the truck he was towing. The employee then loosened the chains, pulled back the lift bar, and walked between the two vehicles. The towed vehicle rolled over the 4x4 block and pinned him against the tow truck. The employee's left leg was amputated and the right leg required surgery. The employee was hospitalized.
