Incident Narrative

An employee was attempting to pull equipment up onto a water tank at a water plant. The guardrail gave away with the equipment that was being hoisted on it and the employee fell 40 feet to the ground below. The employee was hospitalized for fractures to the pelvis and arm as well as a spleen injury.
