Incident Narrative

An employee was adjusting a part while working on a horizontal mill. The part was a 2,000 pound block of metal, connected to a lifting magnet. The block of metal came out of the vise while the employee was using the crane pendant. The employee's left arm was amputated about 4'' below the shoulder socket. The employee was hospitalized.
