Incident Narrative

An employee was opening an oil and gas well. Nitrogen was put into the well to open it and start production. The employee went to the generator to shut it down and an explosion occurred when the cabinet door was opened. The employee sustained burns to over 15% of the body, including the face, neck, arms, and torso that required hospitalization.
