Incident Narrative

An employee had been working on HVAC duct repair on a roof. The employee was descending a fixed ladder on the side of the building when he slipped on the fifth rung and fell approximately 5 feet to the grass. The employee sustained injury to their scapula and fractures to their clavicle and ribs.
