Incident Narrative
An employee was standing on a fork of a walk-behind stacker while holding a half pallet of mustard on the opposite fork. As the forks were raised, the pallet became unstable, causing the employee to lose his balance and fall approximately two feet to the floor. The employee sustained two rib fractures and a punctured lung.
