Incident Narrative
An employee was operating a pallet jack and drove to the truck yard to get tie-down straps out of another trailer. He placed the straps on top on the battery box of the pallet jack. A tie-down strap fell into the steering control box of the pallet jack, causing the vehicle handle to lock up. The employee was pinned between the jack and the trailer, resulting in a pelvis fracture.
