Incident Narrative

An employee was moving pallets of products on the warehouse floor. While repositioning an electric pallet jack, her foot was pinned between the pallet jack and an empty pallet. She was hospitalized with lacerations and fractures to the first, fourth, and fifth toes, requiring surgery.
