Incident Narrative

An employee was on an elevated platform while using a peavy to separate two 16-foot logs that had doubled up on a transfer chain. He attached the peavy to the top log and was leaning back using his body weight as leverage to change the position of the log. As the employee was applying leverage with the peavy to separate the two logs, the peavy slipped off the log and the employee fell backward over the side of the elevated platform. The employee fell approximately 13 feet and suffered fractures to his skull, clavicle, and sternum.
