Incident Narrative
An employee was operating a forklift with a set of floating forks that were attached to the mast. The employee had just delivered a pallet of pipes to the yard and was returning with the fork attachment. The employee lowered the forks to detach from the load bar when they came off incorrectly. He then exited the forklift to manually reset the forklift attachment. The fork set moved and then fell on his lower right leg, resulting in a fractured right tibia and fibula.
